Parent Volunteer Form

I don't usually use a lot of parent volunteers. I have a lot of interest from parents but I've never been able to utilize them in a way that is good for me, them, and the students. I'm slowly involving them more and more each year. I am fortunate to have a lot of interest (sometimes more than I can handle) in helping out. For Open House/Meet the Teacher night I'm setting out this parent volunteer form. It's pretty basic and very general. You'll also see that my school does a background check so I ask for their date of birth. Hopefully some of you will be able to use it!
